介入治疗瘢痕妊娠42例临床分析

目的:探讨双侧子宫动脉介入栓塞治疗子宫瘢痕妊娠的应用价值。方法:收集2007年2月～2009年12月均确诊子宫瘢痕妊娠的患者42例,采用Seldinger法进行右侧股动脉穿刺成功后,将导管超选至双侧子宫动脉,经导管于双侧子宫动脉内灌注MTX后行明胶海绵颗粒栓塞,术后24～48h内行B超引导下清宫术。结果:42例患者均栓塞成功,清宫术中出血量少,血HCG下降理想。结论:介入治疗子宫瘢痕妊娠既安全、有效,又无严重并发症,缩短了住院时间,值得临床推广使用。

Clinical analysis of 42 cases of Cesarean scar pregnancy treated by intervention

Objective:To investigate the application value of bilateral uterine artery intervention embolization used for the treatment of Cesarean scar pregnancy (CSP).Methods:42 cases of patients diagnosed as CSP from 2007.2 to 2010.4, were successfully punctured of right femoral artery by Seldinger method, then the catheter were super chosen to bilateral uterine artery. Gelatin sponge particle was used to embolize after perfusion through bilateral uterine artery with MTX. 24~48 hours after the surgery, complete curettage of uterine cavity was conducted.Results:All the 42 cases were successfully embolized. There was little blood bleeding in complete curettage of uterine cavity. The blood HCG descended ideally.Conclusion:Intervention treatment is a safe, effective method, without serious complications, and has the shortened time in hospital, should be developed in clinical.
