武汉市常住人口和外来人口孕产妇死亡情况分析

目的:分析2003～2009年武汉市孕产妇的死亡情况,为政府部门制定政策提供依据。方法:收集2003～2009年武汉市105例死亡孕产妇的信息,应用SAS V8专业软件进行统计分析。结果:在死亡的孕产妇中,流动人口的年龄比常住人口和转院的孕产妇年龄大,两两相比差异有统计学意义(F=6.40,P=0.002 4);流动人口死亡的孕产妇中,70.37%属于计划外妊娠;死亡的孕产妇在孕期产前检查的次数均不理想;常住人口和流动人口的孕产妇死亡中,有63.27%和85.19%死于直接产科原因,转院来的74.07%的死亡孕产妇死于间接产科原因,直接产科原因最常见的为羊水栓塞和产科出血,间接产科原因最常见的为心血管疾病和肝病;经过评审后发现,59.22%的孕产妇死亡可以避免,21.36%的孕产妇死亡创造条件可以避免。结论:武汉市孕产妇死亡率处于全国较低水平,但孕产妇死亡率下降还存在有一定空间。流动人口孕产妇系统管理和周边县市级医院对高危孕产妇的救治能力有待于加强。

Analysis on the condition of maternal deaths among permanent resident population and floating population in Wuhan

Objective:To analyze the condition of maternal deaths in Wuhan from 2003 to 2009,provide a basis for policy-making department. Methods:The information of 105 maternal deaths in Wuhan from 2003 to 2009 was collected,SAS V8 software was used for statistical analysis. Results:Among the dead pregnant women,the age of floating women was significantly older than those of permanent resident women and transferred women(F=6.40,P=0.002 4);among the dead pregnant women in floating population,70.37% of them were ectopic pregnancy;the frequency of prenatal examination of dead pregnant women was low;63.27% of the dead pregnant women in permanent resident population and 85.19% of the dead pregnant women in floating population died from direct obstetric causes;74.07% of the transferred women died from indirect obstetric causes;the main direct obstetric causes included amniotic fluid embolism and obstetric hemorrhage,the main indirect obstetric causes included cardiovascular diseases and hepatic diseases;59.22% of the dead pregnant women could be avoided,21.36% of the dead pregnant women could be avoided after creating conditions. Conclusion:The maternal mortality in Wuhan is low,but it has room for further reduction;systematical management of floating pregnant women and cure ability of hospitals in circumjacent counties and cities for high risk pregnant women should be enhanced.
