摘    要:目的:在宫颈癌筛查中,联合应用DNA倍体分析和高危型人乳头状瘤病毒(HR-HPV)检测,为临床宫颈癌疑似病例、病理诊断有疑义病例的鉴别诊断提供一种有效的方法。方法:对239例细胞学筛查有异常的病例进行病理组织学检查,随后再次液基薄层细胞学制片、Feulgen染色,做细胞核DNA倍体分析。对有HPV感染的病例,将其检查结果与DNA倍体分析结果进行比较。结果:239例患者中114例进行了HPV检测,其中HR-HPV感染病例55例,出现异倍体69例,二者阳性检出率均与宫颈病变程度相关;HR-HPV感染病例中发现大部分有异倍体产生,二者均不受年龄影响。结论:HR-HPV感染可能导致DNA异倍体的出现,细胞核DNA倍体分析和HPV检查结果又均与病变的程度密切相关,二者联合可为疾病的筛查、预后的判断提供更多有价值的资料。

Functions of DNA ploidy analysis combined with high-risk human papillomavirus detection in screening of cervical cancer
Abstract:Objective:To provide an effective method for differential diagnosis of suspected cases and uncertain cases of cervical cancer by combined application of DNA ploidy analysis and high-risk human papillomavirus (HR-HPV) detection in screening of cervical cancer.Methods:239 cases with abnormal results of cytological screening underwent histopathological examination,then the smears were made by liquid-based cytological technique,stained by Feulgen method,and analyzed by nuclear DNA ploidy analysis;for the cases with HPV infection,the results of HR-HPV detection were compared with those of DNA ploidy analysis.Results:114 cases underwent HR-HPV detection among 239 cases,including 55 cases with HR-HPV infection and 69 cases with aneuploidy,the positive detection rates of HR-HPV infection and aneuploidy were related to degree of cervical lesions;aneuploidy was diagnosed among most of the cases with HR-HPV infection,which didn't be affected by age.Conclusion:HR-HPV infection can lead the emergence of DNA aneuploidy,the results of nuclear DNA ploidy analysis and HR-HPV detection are related to degree of cervical lesions closely,joint detection of DNA ploidy analysis and HR-HPV detection may provide more valuable data for screening and prognosis of cervical cancer.
