| 摘 要： | 目的:研究子宫收缩时脐带绕颈胎儿脐血流的变化,评价其预测脐带绕颈胎儿预后的有效性。方法:对178例单胎足月妊娠脐带绕颈1周的孕妇进行脐血流分析,寻找特异性S/D比值反映胎儿宫内缺氧状态,分析产时S/D比值的变化,比较分析其与不良围产儿的关系。结果:①脐带绕颈组脐血流S/D≥3.0,较其他S/D比值在反映胎儿宫内缺氧方面更有特异性;②产程动态观察,脐带绕颈组胎儿宫缩期比间歇期S/D升高>0.1者胎儿宫内窘迫发生率明显升高,且与无脐带绕颈组比较,脐带绕颈组宫缩期比间歇期S/D值升高>0.1发生率也明显增高,差异有统计学意义。结论:脐血流S/D值测量及宫缩期进行脐血流检测可以对脐带绕颈胎儿预后进行有效评价。
