成人肾透明细胞肉瘤临床病理分析

目的 探讨成人肾透明细胞肉瘤的临床病理特点、免疫组织化学特征及鉴别诊断.方法 应用HE和免疫组化SP法进行vimentin、Bcl-2、CK、EMA、desmin、HHF-35、syn、CgA、S-100蛋白、HMF45、LCA、CD34染色,观察1例成人肾透明细胞肉瘤病理组织学形态及免疫表型特征,并复习文献.结果 镜下见瘤细胞小圆形,胞浆少、淡染或呈空泡状;瘤细胞丰富、致密,弥漫分布,其间掺插的分支状纤维血管间质分隔成巢团状.免疫组化示:瘤细胞vimentin和Bcl-2弥漫阳性,其他上皮性、肌源性、神经源性等标记均为阴性.结论 肾透明细胞肉瘤是成人罕见的恶性肾肿瘤,诊断主要依靠组织病理学分析和免疫表型的检测.

Adult Clear Cell Sarcoma of the Kidney:a case Report and Clinical Pathologic Analysis

Objective To study the clinicopathological and immunohistochemical features, and differertial diagnosis of clear cell sarcoma of kidney in adults. Methods An adult Clear cell sarcoma of kidney was investigated by means of light microscopy, HE stain, immunohistochemical stain for vimentin,Bcl- 2, CK, EMA, desmin, HHF-35, syn, CgA, S-100 protein, HMF45, LCA, CD34, together with review of the related literature. Results Microscopically, the tumor showed characteristic malignant features. It consisted of nests of small round tumor cells with scanty and light staining or vacuolus cytoplasm surrounded by fibrotic blood vessel stroma. The abundant, densely tumor cells distributed in diffuse. Immunohistochemical stains were strongly diffuse positive for vimentin and Bcl-2 and negative for all of other antigens. Conclusions CCSK is a rare type of malignant renal neoplasm in adults and its diagnosis bases mainly on histopathological analysis and immunophenotypic detection.
