| 摘 要： | 目的探讨血乳酸水平结合中心静脉压监测在复杂先心病术后治疗中的应用价值,拟为术后处理提供参考依据。方法选择沈阳军区总医院心外科2007年6月—2007年12月收治的60例复杂先天性心脏病婴幼儿,随机分为两组:中心静脉压组(Ⅰ组);血乳酸结合中心静脉压组(Ⅱ组)。观察术后呼吸机辅助时间,肺内感染的发生率,住院时间,死亡率等指标。结果Ⅰ组:术后呼吸机辅助时间(31.0±5.5)h,肺内感染发生率7%,平均住院时间(21.0±3.5)d,死亡率为6%。Ⅱ组:术后呼吸机辅助时间(16.0±5.5)h,肺内感染发生率3%,平均住院时间(15.0±2.4)d,死亡率为2%。Ⅱ组即乳酸指导治疗组死亡率和平均住院时间,肺炎发生率,使用呼吸机时间均有明显改善(P<0.05)。结论血乳酸水平结合中心静脉压在复杂先心病术后治疗中对于补液及心功能的调整比单独监测中心静脉压更有优势,更好地监测组织水平的灌注,改善患儿的预后。
