胰胆管十二指肠连接区癌的多层螺旋CT诊断

目的探讨多层螺旋CT（MSCT）增强扫描及后处理技术在胰胆管十二指肠连接区（PDDU）癌中的诊断及鉴别诊断价值。方法回顾性分析38例有完整CT检查资料,经手术及病理证实为PDDU癌的CT表现。结果壶腹癌15例,胰头癌17例,胆总管下端癌5例,乳头周围区十二指肠癌1例,全部表现为低位胆道梗阻。MSCT多期增强扫描及后处理技术的运用可以进一步显示病灶的位置、大小、与周围组织关系、血供情况、周围脏器及淋巴结转移情况。结论MSCT的多期增强扫描及后处理技术的运用,可以提高PDDU癌的诊断、鉴别诊断水平,对PDDU癌临床术前评估具有重要的指导意义。

Multi-slice CT Diagniosis of Carcinomas in Pancreaticobiliary Ductal and Duodenal Union

Objective To determine the value of multi-slice CT（MSCT）in the diagnosis of carcinomas occurred in（PDDU）.Methods Features on MSCT of thirty-eight cases with PDDU carcinoma verified by pathology were retrospectively analyzed.Results All cases （15 ampullary carcinomas,17 carcinomas of head of pancreas,5 carcinomas of distal common bile duct,1 carcinoma induodenal papilla） presented with lower obstruction of bile duct.Multi-phase enhanced MSCT with multi-plannar reconstruction（MPR） and threedimensional CTangiography（3D-CTA）clearly showed the tumors’ site,size,infiltration,blood supply and metastasis.Conclusion Multi-phase enhanced MSCT with post processing technique image is helpful in diagnosis and differential diagnosis of carcinoma of PDDU, and is important in preoperative evaluation.
