消化道出血的血管造影诊断与介入治疗的相关问题探讨

目的探讨消化道出血的血管造影诊断与介入治疗的临床应用价值和影响诊疗的因素。方法对45例临床确诊为消化道大出血患者,采用选择性DSA检查,明确出血部位和出血动脉后采用栓塞治疗或注射加压素治疗。结果 45例中37例发现出血征象,阳性率为82.22%（37/45）。对其中31例进行栓塞治疗,栓塞治疗有效率为87.10%（27/31）;6例注射加压素治疗,其中2例止血成功。所有患者均未发生严重并发症。结论 DSA不仅可明确诊断消化道出血,还可同时进行介入治疗,止血效果肯定。

Angiographic diagnosis and related factors of interventional therapy for digestive tract hemorrhage

Objective To investigate the angiographic diagnosis and related factors of interventional therapy for digestive tract hemorrhage. Methods Totally 45 patients clinically diagnosed as digestive tract hemorrhage underwent selective angiography, the location of hemorrhage and bleeding arteries were ascertained. Then embolization or urotensin infusion through selective indwelling catheter was performed. Results The bleeding arteries were found and located in 37 of 45 patients, the positive ratio was 82.22% (37/45). Of the 37 patients, embolization was performed on 31 and was successful in 27 (27/31, 87.10%), while 6 patients were treated with urotensin infusion and 2 were successful. No serious complications happened. Conclusion Selective angiography for upper digestive tract hemorrhage can not only show the bleeding artery and location, but also allow embolization at the same time and its effect is sure.
