宫颈涂片与液基细胞学筛查宫颈癌的荟萃分析

目的:比较分析常规宫颈涂片与液基细胞学检查方法对宫颈癌筛查的优异度。方法:通过数据库检索2003～2012年问国内外公开发表的14篇随机对照文献,提取36038例常规涂片样本和40772例液基细胞学样本,利用meta分析方法处理相关数据,并对其若干指标进行对比。结果:与常规宫颈涂片检查相比,液基细胞学对宫颈癌筛查的阳性检出率OR=2.70,95%CI(1.52,4.81)p<0.05],标本满意率OR=1.33,95%CI(1.04,1.70)p<0.05],诊断符合率OR=1.59,95%CI(1.30,1.94)P<0.05],敏感度OR=3.57,95%CI(1.33,9.55)p<0.05]及检验成本MD=0.61,95%CI(0.14,1.08)p<0.05],差异有统计学意义,而特异度OR=1.42,95%CI(0.60,3.55)p>0.05],差异没有统计学意义。结论:液基细胞学检查筛选宫颈癌,在阳性检出率、标本满意率、诊断符合率、敏感度和均优于常规宫颈涂片检查。

Meta-analysis on Cervical Cancer of Cervical Smear and Liquid Base Cytology Screening

Objective Comparative analysis of conventional cervical smear and liquid, base cytology inspection method for cervical cancer screening high degree. Methods Through the database retrieval 2003-2012 published at home and abroad of 14 article randomized controlled literature, extraction 36038 cases of conventional smear samples and 40772 cases of fluid base cytology sample, using meta analysis method to deal with related data, and its several index were compared. Results Compared with conventional cervical smear, liquid base cytology on cervical cancer screening positive detection rate 0R=2.70, 95%CI （ 1.52, 4.81 ） p〈0.05], specimen satisfaction OR = 1.33, 95% CI （1.04, 1.70） p〈0.05], diagnosis coincidence rate 0R=1.59, 95%CI （1.30, 1.94） p〈0.05], sensitivity OR = 3.57, 95% CI （1.33, 9.55） p〈0.05] and inspection cost MD= 0.61, 95% CI （0.14, 1.08） p~0.05], the difference was statistically significance, and specific degrees OR = 1.42, 95% CI （0.60, 3.55 ） p〉0.05], the difference was not sta- tistically significant. Conclusion liquid base cytology screening cervical cancer, in the positive detection rate, specimen satisfaction rate, diagnostic coincidence rate, sensitivity and test cost is superior to conventional cervical smear, both in the degree of specific no statistical significance.
