MR灌注成像在兔脊柱结核早期诊断中的实验研究

摘    要:目的:探讨磁共振灌注成像(MRI-PWI)在兔早期脊柱结核血流灌注动态化监测中的应用价值。方法:选择30只健康新西兰大白兔,于腰6椎体横突钻孔注入结核杆菌制备脊柱结核动物模型,在手术后4、6、8周分别采用常规MRI及PWI进行扫描,观察常规MRI表现及病变椎体的血流灌注变化,选择感兴趣区(ROI),由计算机自动生成时间一信号曲线,得出相对信号强度增加率ASI,比较不同时间ASI增加率的差异,分析其血流灌注的特点,并进行统计学分析。结果:成功建立脊柱结核动物模型。脊柱结核动物模型术后第4、6、8周椎体及椎间盘形态未见明显异常,但常规MR出现异常信号,术后8周病变区与正常区域血流灌注值的相对信号强度增加率ASI的差异具有统计学意义(P〈0.05)。结论:磁共振灌注成像(PWI)能够评估早期脊柱结核血流灌注。选择适合的感兴趣区(ROI)能较准确的判定病变区的血流灌注情况。

Abstract:Purpose: To discuss applicated value of perfusion weighted imaging in rvasc ular monitoring of rabbit's spinal tuberculosis. Methods: 30 healthy New Zealand white ra bbits were selected in this study.Respectively animal models of spinal tuberculosis were made by Surgery operation. Then they were examined with common MRI and PWI at 4week 6weeks, 8weeks respectively. The model of bone graft district as the research object, choo se region of interest (ROI) respectively, computer can automatic generate of time-signal cu rye, compare the signal strength increasing rate of different time and analyzes its character istics of blood perfusion. Results: The spinal tuberculosis model was successfully established in rabbits. Vertebral and intervertebral disc has not seen the apparently unusual at 4wee ks, 6weeks, 8weeks. But common MRI and DWI Appear unusual signal.The graft rate of i ncreae of the relative signal strength/k SI statistically significant differencebetween4week and 8weeks, 6weeks and 8weeks (P 〈0.05). Conclusion:Perfusion-weighted MR imaging is useful to evaluateblood perfusion in the early spinal tuberculosis. Se- lect the proper interes ted region (ROI) can be accurately determine the perfusiongrafts.
