超声血管增强技术对中青年脂肪肝患者颈动脉硬化的评价

摘    要:目的探讨超声血管增强技术(ultrasoundvascularenhancementtechnology,VET)对中青年脂肪肝患者颈动脉检测的临床价值,分析脂肪肝与颈动脉硬化之间的关系。方法对经腹部超声诊断的50例中青年脂肪肝患者(脂肪肝组)和同期腹部超声排除脂肪肝的志愿者50例(对照组)进行颅外段颈动脉(颈总动脉、颈内动脉起始段、颈外动脉起始段)彩超VET检查,观察其内中膜厚度(IMT)和斑块的形成情况,并与传统二维超声图像对比。结果脂肪肝组患者的颈动脉IMT较对照组增厚(P〈0.05),动脉硬化斑块的发生率增高。与传统二维声像图比较,VET可清晰显示颈动脉管壁结构及斑块轮廓。结论VET可早期发现脂肪肝患者的颈动脉病变,对其及早进行临床干预,可减少患者预期血管并发症的发生,具有一定的临床指导意义和应用价值。

Abstract:Objective To study the diagnostic value of ultrasound vascular enhancement technology (VET) in the detection of carotid atherosclerosis in young and middle - aged patients with fatty liver and to explore the relationship between fatty liver and carotid atherosclerosis. Methods 50 young and middle - aged patients with fatty liver diagnosed by abdominal ultrasonography were selected as fatty liver group and 50 young and middle - aged volunteers without fatty liver were selected as control group. Extracranial carotid artery ( including common carotid artery, the initial segment of internal carotid artery and external carotid artery) was examined by color Doppler ultrasound VET for carotid intima -media thickness (IMT) and formation of plaque and then compared with traditional two - dimensional ultrasonography. Results Compared with control group, carotid IMT in fatty liver group was significanly increased ( P 〈 0.05 ), with increased incidence of atherosclerotic plaques. Compared with traditional two - dimensional ultrasonography, VET could clearly display the contours of plaque and structure of carotid artery. Conclusion VET can be used to detect early stage of carotid atherosclerosis in patients with fatty liver in order to provide guidence for early clinical intervention for reduction of expected vascular complications. VET has certain value for clinical application.
