摘    要:目的探讨肺功能在上气道梗阻(UAO)诊断中的价值及引起UAO的主要原发病。方法测定UAO患者肺功能,按肺功能流速-容量曲线形态分组,分析28例UAO患者原发病因及肺功能资料。结果胸内型及固定型UAO呼气相流速受限明显,此两组患者FVC、FEV1、FEV1/FVC、PEF、FEF25%较胸外型UAO显著下降(P〈0.05),胸外型UAO及固定型UAO表现为吸气相流速受限,FEF50%/FIF50%比值在三组间均差异显著(P〈0.05),而三组间VC无明显差异(P〉0.05)。结论肺功能对UAO的评价应按不同类型分别判断,肺功能参数的仔细判读对UAO的诊断有重要意义。

Abstract:Objective To explore the value of lung function testing in upper airway obstruction diagnosis.Methods Lung function testing was performed on 28 patients with upper airway obstruction (UAO).According to the shape of flow-volume loop,28 patients were divided into three groups.Lung function parameters and primary etiological factors were studied.Results Expiratory flow limit was found in intrathoraxic UAO and fixed UAO.The values of FVC,FEV1,FEV1/FVC,PEF,FEF25% in both UAO groups were obvious lower than those in the extrathoraxic UAO (P〈0.05).Inspiratory flow limit was found in the extrathoraxic UAO and fixed UAO.The values of FEF50%/FIF50% was obviously different in the three groups (P〈0.05).While VC was not obvious different in the three groups (P〉0.05).Conclusions Evaluation of UAO by pulmonary function testing should be assessed by different obstructive types.Analysis of lung function parameters has a considerable value in the UAO diagnosis.
