全髋关节置换术后疼痛的临床分析及治疗

目的探讨人工全髋关节置换术后疼痛的原因及临床治疗方法。方法 1997年8月-2010年10月我院收治的各类髋关节疾病患者82例(87髋),首次均行人工髋关节置换术(87髋),术前及第一、第二次术后(末次随访时)行Harris评分,分析比较术前、术后评分并进行统计学分析。结果 82例患者中有10例(11髋)出现不同程度的疼痛,无疼痛组(72例)Harris评分由术前的(35.2±4.9)分提高到末次随访的(90.5±2.4)分,效果满意。疼痛组(10例)保守治疗4例,末次随访Harris评分(87.5±2.1),与治疗前(55.3±3.6)分相比较差异有统计学意义(P

After total hip arthroplasty clinical analysis and treatment of pain

Objective To explore the cause and clinical treatment methods of post total artificial hip arthroplasty pain.Methods Collecting the data of patients with variety of hip diseases visiting our hospital,mounting up to 82 patients (87 hips),from August1997 to October 2010.All patients were given total artificial hip arthroplasty,Harris score was made pre and post the first,second(last follow up) operation,and the scores were compared for statistical analysis.Results In all patients,10 (11 hips) with varying degrees of pain.The group without pain (72 cases) Harris score was elevated from preoperative (35.2±4.9) points to the last follow-up (90.5±2.4),and the result was satisfactory.The group with Pain (10 cases):4 conservative treatments,the last follow-up Harris hip score (87.5±2.1) compared with pre-treatment (55.3±3.6) points,difference was statistically significant (P<0.05);6 surgical treatments,the last follow-up Harris hip score (91.2±2.6) points compared with the second preoperative (56.0±4.3) points,difference was statistically significant (P <0.05).The pain disappeared in all patients,and X-ray assessment showed no loosening of the prosthesis.Conclusion Careful preoperative preparation,intraoperative skilled,scientific,rigorous operation,strict aseptic concept and technique is the key to prevent groin pain,and proper rehabilitation exercise is an important measure to reduce postoperative pain.The second repair surgery of the hip joint is an important method to solve the first hip postoperative pain.
