风湿性心脏病并发房颤患者在围手术期中的复律研究

目的：探讨风湿性心脏瓣膜病并发房颤患者围手术期电击结合口服胺碘酮在复律中的应用价值。方法：围手术期口服胺碘酮，术中瓣膜置换后，根据心脏自动复跳情况采用电击复律。结果：关胸后、术后3d、10d、3个月分别有93．3％、80．0％、73.3％、66．7％的患者维持窦性心律。结论：该方法复律安全、效果好，可提高房颤的转复率，减少术后复发率。

Cardioversion during perioperative period in rheumatic valvular heart disease

Objective:To deermine the value of direct current cardiov er sion (DCC)on cardiac surface in cardiac operation with amiodarone used on pati ents of reumatic heart disease accompanied with atrial fibrillation (AF)during perioperation period.Methods:30patients were divided into two groups rando mly,matched by sex,age and course of AF.The treatment group took amiodarone orally during perioperative period,but not used at the operation day.In opera tion,after the valve was re-placed,cardioversion was delivered to the patient based on the status of the heart.The control group were dealed as same as th e treatment group except not being treated with amiodarone.Results:93.3 %,80.0%,73.3%,66.7%of respectively patients remained sinus rh ythm after closure of thorax and3,10,90days after operation,in contro gr oup,the percentages were40.0%,26.7%,20.0%and13.3%,respec-tively.Con clusion:The method was proved to be safe and efficient for cardi oversion and mentainance of sinus rhythm.
