胰腺炎并发网膜囊脾隐窝及胃脾韧带区假性囊肿的CT诊断

目的:探讨网膜囊脾隐窝及胃脾韧带区胰源性假性囊肿形成的解剖学基础、病理机制及CT诊断价值。材料和方法:回顾性分析15例网膜囊及胃脾韧带区胰源性假性囊肿的CT征象。结果:8例网膜囊区假性囊肿有2例延伸到网膜囊脾隐窝,6例胃脾韧带区假性囊肿均与脾门、胰尾部病变有联系,1例为胰周多发囊肿。结论:脾肾韧带及胃脾韧带的连续性是胃脾韧带区胰源性假性囊肿形成的解剖学基础;囊肿与脾门是否连续是鉴别网膜囊脾隐窝与胃脾韧带区胰源性假性囊肿的重要依据;CT检查对胰源性假性囊肿的诊断、定位及其动态监测具有重要临床意义。

Diagnosis of Pancreatitis Associated with Pseudocysts in Omental Bursa and Gastrosplenic Ligament

Purpose:To explore the anatomical basis and pathological mechanism of pancreatitis associated with pesudocysts,and their CT diagnosis value.Materials and Methods: The CT sfeatures of 15 cases with pancreatitis associated with pseudocysts in the omental bursa and gastrosplenic ligament were analyzyed retrospectively and their anatomical basis were studied.Results: Among 15 cases,there were 8 pesudocysts in the omental bursa,6 in gastrosplenic ligament related to the foci in splenic hilus and pancreas,and 1 multi-pseudocyst in peripancreas.Conclusions: The relationship between splenorenal ligament and gastrosplenic ligament is a important anatomic basis of the pancreatic pesudocysts in gastrosplenic ligament.The continuity between pseudocyst and splenic hilus is an important sign for differential diagnosis pseudocysts in omental bursa from one in the gastrosplenic ligament.CT scan is of important value for diagnosis,localization and dynamic monitoring of pancreatic pesudocysts.
