卡前列素氨丁三醇联合缩宫素治疗宫缩乏力性产后出血的效果观察

目的探讨卡前列素氨丁三醇联合缩宫素治疗宫缩乏力性产后出血的临床效果。方法选择本院2011年1月-2014年1月收治入院的宫缩乏力产后出血患者200例临床资料进行回顾性分析,按照治疗方法不同分为两组,每组100例,观察组在胎盘娩出后给予子宫体注射缩宫素20 U＋卡前列素氨丁三醇250μg（根据病情可以重复使用,间隔必须15 min以上）,对照组给予米索前列醇200μg直肠放置,观察两组的宫缩强度和出血量。结果观察组的总有效率明显高于对照组,差异有统计学意义（χ^2=9.308,P〈0.05）;观察组产后5、10、30、60 min四个时间段宫缩强度均强于对照组,差异有统计学意义（P〈0.05）;观察组产后2、8、24 h的出血量均少于对照组,差异有统计学意义（P〈0.05）。结论卡前列素氨丁三醇联合缩宫素能有效治疗宫缩乏力产后出血。

Effect observation of hemabate combined with oxytocin in the treatment of postpartum hemorrhage caused by uterine inertia

Objective To investigate clinical effect of hemabate combined with oxytocin in the treatment of postpartum hemorrhage caused by uterine inertia. Methods Clinical data of 200 patients with postpartum hemorrhage caused by uterine inertia in our hospital from January 2011 to January 2014 were retrospectively analyzed and divided into two groups according to the treatment method,each group of 100 cases.After delivery,the maternal uterine body injection of oxytocin 20 U＋hemabate 250 μg（it can be repeat according to the illness,must be more than 15 minutes））were used in observation group.Misoprostol 200 μg by rectal contraction placed was applied in control group.Contraction intensity and the amount of bleeding in two groups were observed. Results Total effective rate of observation group was obviously higher than that of control group,with statistical difference（χ^2=9.308,P〈0.05）.Contraction intensity of postpartum5,10,30,60 min of observation group was strongger than that of control group respectively,with statistical difference（P〈0.05）.The amount of bleeding of postpartum 2,8,24 h in observation group was less than that in control group respectively,with statistical difference（P〈0.05）. Conclusion Hemabate combined with oxytocin can be effective in the treatment of postpartum hemorrhage by uterine inertia.
