机械辅助排痰对危重患者人工气道气囊压力及呼吸功能的影响

摘    要:目的探讨机械辅助排痰对危重患者人工气道气囊压力及呼吸功能的影响。方法选取本院2013年5月-2014年5月收治的50例建立人工气道的危重患者作为研究对象,比较机械辅助排痰前后人工气道气囊压力的变化、呼吸次数、Sp O2、Pa O2和不良反应发生情况。结果排痰后的平均气囊压力显著低于排痰前,差异有统计学意义(P〈0.01)。排痰后的呼吸次数和不良反应发生率显著低于排痰前,差异有统计学意义(P〈0.05)。排痰后的Sp O2、Pa O2水平显著高于排痰前,差异有统计学意义(P〈0.05)。结论机械辅助排痰能够改善建立人工气道患者的呼吸功能,但由于排痰后气囊压力会受到影响,因此排痰后必须监测气囊压力。

Abstract:Objective To explore the influence of mechanical assisted expectoration on the artifical airway ballonet pressure and respiratory function of patient with critical illness. Methods 50 critical illness patients with artifical airway from May 2013 to May 2014 were seleced as the research objects.The changes of artifical airway ballonet pressure,RR,Sp O2,Pa O2 and the incidence of adverse reaction events before and after mechanical assisted expectoration was compared. Results Average artifical airway ballonet pressure after expectoration was lower than that before expectoration,with significant difference(P〈0.01).Respiratory rate and the incidence of adverse reaction fuction after expectoration was better than that before expectoration,with significant difference(P〈0.05). The level of Sp O2,Pa O2 after expectoration was higher than that before expectoration,with significant difference(P〈0.05). Conclusion Mechanical assited expectoration can improve respiratory function of patients with artificial airway,but because ballonet pressure is affected after expectoration,so the ballonet pressure must be monitored after expectoration.
