妊娠肝内胆汁淤积症胆酸与EPO变化及其临床意义

摘    要:目的 观察妊娠期肝内胆汁淤积症(ICP)孕妇静脉血、羊水、脐血中胆酸、促红细胞生成素(EPO)变化及其相关性研究,探讨其临床意义. 方法 对52例ICP患者采用酶联免疫吸附试验(ELISA)和放免法(RIA)测定孕妇血、羊水、脐静脉血中胆酸及EPO水平;另选取46例正常足月妊娠孕妇作对照组;胎儿娩出时抽取脐动脉血进行血气分析以判定是否缺氧.结果 ICP组孕妇血、脐静脉血及羊水中胆酸水平显著高于对照组;ICP组孕妇血EIK)水平同对照组相比差异无统计学意义(P>0.05),ICP组妇女的胎儿脐静脉血及羊水中EPO含量均明显低于对照组胎儿的水平(均P<0.05),ICP组中缺氧胎儿的脐静脉血EPO水平明显低于未发生缺氧胎儿的水平,ICP组孕妇血、脐静脉血及羊水中胆酸与脐静脉血、羊水的EPO分别成负相关. 结论 ICP孕妇血清高胆酸水平对胎儿体内合成EPO有一定影响;胎儿EPO水平低下是其宫内缺氧的原因之一;检测羊水EPO可以作为产前监测ICP胎儿官内缺氧的一项参考指标.
