生脉注射液对直立性低血压疗效的短期观察

目的观察生脉注射液对直立性低血压的短期疗效。方法 76例继发性直立性低血压患者分为常规治疗组(n=40)和生脉组(n=36)。常规治疗包括非药物治疗和扩容治疗,生脉组在此基础上,加用生脉注射液静脉滴注。监测卧立位血压变化和24 h动态血压变化。用头晕障碍量表(DHI)评估症状的变化。结果卧立位血压测量,常规治疗组患者治疗前血压下降(18.7±4.6)/(7.2±3.5)mmHg,治疗后血压下降(15.8±4.1)/(6.3±2.7)mmHg(P0.05);生脉组患者治疗前血压下降(19.8±5.0)/(8.1±3.3)mmHg,治疗后血压下降(15.1±4.8)/(6.5±2.4)mmHg(P0.01)。24 h动态血压监测平均血压,常规治疗组上升(4.4±2.7)/(2.8±2.0)mmHg(P0.05);生脉组上升(6.8±3.3)/(3.0±2.5)mmHg(P0.01)。DHI评分常规治疗组减少(8.7±4.1)分(P0.05);生脉组减少(15.8±3.3)分(P0.01)。结论生脉注射液可以轻度升高血压,改善直立性低血压患者的症状,可用于直立性低血压的辅助治疗。

Short-term clinical efficacy of Shengmai injection on orthostatic hypotension

Objective To evaluate the short-term efficacy of Shengmai injection in the treatment of orthostatic hypotension(OH).Methods All 76 patients with secondary OH were randomly divided into two groups: conventional therapy group(n=40)and Shengmai injection group(n=36).Conventional therapy included non-drug treatment and volume expansion.Beside conventional therapy,patients in Shengmai injection group were received Shengmai injection for intravenous drop.Supine and standing BP measurements and ambulatory BP monitoring(ABPM) were used to assess the alteration of BP.The symptoms were assessed by Dizziness Handicap Inventory(DHI).Results The fall of BP in supine and standing BP measurements were 18.7±4.6/7.2±3.5mmHg before treatment and 15.8±4.1/6.3±2.7mmHg after treatment in conventional therapy group(P>0.05);19.8±5.0/8.1±3.3mmHg before treatment and 15.1±4.8/6.5±2.4mmHg after treatment in Shengmai injection group respectively(P<0.01).An elevation of average BP in ABPM was 4.4±2.7/2.8±2.0mmHg in conventional therapy group(P>0.05) and 6.8±3.3/3.0±2.5mmHg in Shengmai injection group respectively(P<0.01).A significant reduction of DHI scores was observed in Shengmai injection group(P<0.01).Conclusion Shengmai injection therapy might elevate BP slightly,reduce the fall of BP in supine and standing BP measurement,improve the symptoms of patients with OH and can be severed as an adjuvant therapy for OH.
