IVF与ICSI周期实验室及临床结果观察

目的比较常规体外受精-胚胎移植(IVF-ET)与卵胞浆内单精子注射(ICSI)两种助孕技术的临床妊娠结局及产科结局。方法对843个IVF-ET及ICSI治疗周期的流产、异位妊娠、多胎妊娠的发生率及新生儿出生体重、胎儿孕龄、先天性畸形的情况进行回顾性分析;并对两组单胎、双胎妊娠的结局分别进行比较。结果ICSI组的受精率显著高于IVF组,IVF和ICSI两组早期胚胎的发育情况、产科结局、围生情况及出生后婴儿的健康情况差异无显著性(P>0·05)。但是双胎妊娠和单胎妊娠两组低体重儿、小于胎龄儿、早产的发生率之间有显著性差异,双胎妊娠组低体重儿、小于胎龄儿、早产的发生率均明显高于单胎妊娠组(P<0·05)。结论IVF与ICSI两组的妊娠结局、围生儿结局相似;ICSI不增加胎儿畸形率及新生儿并发症几率,双胎妊娠是引起低体重儿、小于胎龄儿及早产的主要原因。

Study on comparation of pregnancy outcome between conventional in vitro fertilization and intracytoplasmic sperm injection

Objective To compare the pregnancy outcome between conventional in vitro fertilization embryo transfer(IVF-ET) and intracytoplasmic sperm injection(ICSI).Methods A study was carried out to analyse clinical abortion,ectopic pregnancies,multiple gestations,birth weight,gestational age,congenital malformation in patients receiving either IVF or ICSI from January 2004 to October 2005 in the Assisted Reproductive Centre of the first Affiliated Hospital of Anhui Medical University.The outcomes of singleton group and twin group were compared separately.Results The fertilization rate of ICSI group was significantly higher than IVF group(P<0.05).There were no significant differences in early embryonic development,obstetric outcomes,perinatal situation and neonatal health condition between IVF group and ICSI group.While there were significant differences in the rates of low birth weight,small for gestational age and pre-term birth between singleton group and twin group.These rates were significantly higher in twin group than in singleton group(P<0.05).Conclusions The pregnancy and perinatal outcomes are similar between IVF group and ICSI group.The ICSI do not increase the risk of major malformation and neonatal complications comparing with IVF.And twin is the main cause of low birth weight,small for gestational age and pre-term birth.
