彩色多普勒超声对缺血性脑血管病干预治疗颈动脉内中膜及斑块的动态检测

摘    要:目的:探讨彩色多普勒超声动态检测颈动脉内中膜(intima-mediathictmess,IMT)及斑块变化对缺血性脑血管病的临床应用价值。方法:应用彩色多普勒超声检测99例缺血性脑血管病患者干预治疗前后颈动脉IMT及斑块变化,并与对照组比较分析。结果:缺血性脑血管病组IMT明显高于对照组;随年龄增长,IMT有增厚趋势;斑块(尤其多发斑块)发生率、软斑和混合斑发生率明显高于对照组;分叉处(bifurcation,BIF)斑块发生率明显增高;IMT〉1.0mm合并低密度脂蛋白(thelowdensity lipoprotein,LDL)增高者经临床干预治疗后IMT明显降低,部分斑块消退。结论:彩色多普勒超声检测颈动脉可早期发现并动态观察颈动脉粥样硬化(carotid artery athemsclemsis,CAS)的发生发展过程,并能为缺血性脑血管病的早期干预治疗提供重要的参考价值。

Abstract:Objective:To evaluate the worthy of carotid artery intima-media thickness (IMT) and the plaques in ischemic cerebrovascular diseases using color Doppler ultrasound. Methods: The arteries of 99 patients with ischemic cerebrovaseular diseases were measured with color doppler ultrasound, and 30 healthy cases were selected as the contrast group. Results: The IMT of the ischemie cerebrovaseular group were higher than that of the normal group ( P 〈 0.05) ; there were obvious differences among the different ages group ( P 〈 0.05) ; the occurrence rate of the plaques, especially the soft and admixture ones of the ischemie cerebrovascular group was higher than that of the normal group ( P 〈 0.05) ; the occurrence rate of the plaques at the bifurcation (BIF) was higher than any other locations ( P 〈 0.05) ; and the IMT which was thicker than 1.0 mm with higher level low density lipoprotein (LDL) became lower by early interruption ( P 〈 0.05), and some of the plaques disappeared. Conclusion: With the technology of color doppler ultrasound, we can observe the occurrence and the development of carotid artery atherosclerosis at an early stage. Therefore, it is very important for early diagnosis and treatment of ischemic cerebrovascular patients.
