冠状动脉病变临床高危因素相关研究

摘    要:目的判断川崎病患儿是否处于冠状动脉病变发生的高危状态,以利于对患儿能更早期诊断,更个别化治疗。方法分别记录两组患儿的年龄、性别、发热持续时间,检测相关的实验室指标,行心脏超声心动图并进行对比分析。结果88例川崎病患儿,40例存在有冠状动脉病变(Cal),发生率为45.45%,3例为巨大冠状动脉瘤(3.4%),均为男性。统计结果显示:发热持续时间、年龄<1岁、白细胞总数(WBC)、低血清白蛋白、低血钾、低血磷,(P<0.05),有统计学意义。性别统计分析无统计学差异。结论发热持续时间、年龄<1岁、高白细胞总数(WBC)、低血清白蛋白、低血钾、低血磷是冠状动脉病变发生的高危因素。临床医师应综合判断,才能提高川崎病诊断率,早期治疗,有效防治冠状动脉病变的发生。

Abstract:Objective To judge whether children patients with Kawasaki disease are in great danger of pathological changes in coronary artery,in order to diagnose promptly and treat individually. Methods The age,gender,duration of high temperature were recorded separately,and relative laboratory indexes were detected.The patients undertake echocardiogram.The contrast analysis was studied. Results Forty of 88 children patients with Kawasaki disease had pathological changes in coronary artery,incidence rate was 45.45%,and 3 was giant coronary artery aneurysm(3.4%),who were all male.The statistical results showed there was significant difference in duration of fever,age<1 year,high WBC,low serum albumin,hypokalemia,hypophophatemia.But there was no significant difference in gender. Conclusion Duration of fever,age<1 year,high WBC,low serum albumin,hypokalemia,hypophophatemia are all high risk factors for pathological changes in coronary artery.Clinicians should judge synthetically to elevate diagnosis rate of Kawasaki disease.And the patients should be treated early to prevent development of pathological changes in coronary artery effectively.
