现代影像诊断学在累犯血管的软组织肉瘤外科治疗中的应用

目的:探讨现代影像学在软组织肉瘤累犯血管的诊断价值和对外科治疗的指导意义.方法:4例软组织肉瘤手术前、后均行影像学检查.结果:平均随访6个月,肿瘤无复发.现代影像学诊断血管被侵犯后的手术干预率为87.5%.结论:CT、MRI、DSA对诊断软组织肉瘤是否侵犯血管有重要价值,并能评价受累程度,从而指导外科治疗.

The Application of Modern Imageology in Surgical Treatment of Soft Tissue Sarcomas

Objective: To evenluate the role of CT, MRI and DSA in assessment of the extent and severity of the invaded vessels related to a soft tissue sarcoma(STS), in order to provide available informations for operation planning. Methods: Radiographic (in 4 cases), CT (in 4 cases), MRI (3cases) and DSA (in 1 case) findings obtained preoperatively in 4 STS patients verified on operation were analysed. After operation, the local circulation was evaluated with Doppler (in 4 cases), CT and USG (3 cases). Results: CT, MRI and DSA demonstrated that 8 major vessels were invated by the sarcoma. Of them, one was very close to the tumor; 2 were adhered to the tumor, and 5 were mingled with the tumor. Seven vessels were operated upon including artery sheath resection 7, all vessels resection with GORE-TEX prosthesis transplant (in 5 cases). The surgical margins was histologically normal in 7 cases. The rate of surgical intervention was 87.5% in cases with invaded vessels demonstrated by CT, MRI and DSA. Conclusions: CT, MRI and DSA are valuable in assessment of the extent and severity of invaded vessels related to a soft tissue sarcoma(STS), thus providing useful information for operation planning.
