宫颈环形电切术治疗宫颈病变266例临床疗效观察

目的探讨宫颈环形电切术（LEEP）治疗各种宫颈疾病的临床疗效。方法对妇科门诊常规检查初步诊断为各种宫颈疾病如慢性宫颈炎（宫颈糜烂、宫颈息肉、宫颈肥大、宫颈纳氏囊肿、宫颈湿疣等）、宫颈上皮内瘤样变（CINⅠ、CINⅡ、CINⅢ级）等的266例患者,采用环形电刀行宫颈环切术治疗,手术标本送病理检查,并观察手术时间、术中出血量、患者反应、病理检查及术后随访情况。结果 LEEP治疗宫颈疾病成功266例（100%）,手术时间（10±4）min,出血量（10.0±2.5）ml,仅12例（4.51%）患者出现下腹隐痛、阴道烧灼感;病理检查术前、术后符合率,慢性宫颈炎96.23%,宫颈上皮内瘤变CINⅠ～Ⅱ级100%,CINⅢ级90.00%,术后随访6个月均无复发情况,亦未出现术后感染、大出血及宫颈口粘连、宫颈口狭窄等并发症。结论 LEEP具有快速简便、并发症少、手术时间短、出血少、患者痛苦小等优点,在宫颈疾病治疗中安全有效,具有诊断及治疗双重疗效,是治疗宫颈病变较理想的方法,并且不需住院,大大减少了治疗费用,值得临床广泛推广应用。

Cervical loop electrosurgical excision procedure for cervical lesions in 266 cases

Objective To investigate the clinical efficacy of cervical loop electrosurgical excision procedure（LEEP） in the treatment of cervical disease.Methods Preliminary examination of routine gynecological outpatient diagnosis of cervical diseases such as chronic cervicitis（cervical erosion,cervical polyps,cervical hypertrophy,cervical Nessler cysts,cervical warts,etc.）,changes of cervical intraepithelial neoplasia（CIN Ⅰ-Ⅲ）,etc.Two hundred and sixty-six patients in the treatment of LEEP and surgical specimens were sent to pathology.To observe the operation time,peroperative bleeding,patient response,pathological examination and follow-up the situation of post-operative.Results Two hundred and sixty-six patients（100%） were cured after treatment of LEEP.The operative time was（10±4）min,the peroperative bleeding was（10.0±2.5）ml,only 12 patients（4.51%）had abdominal pain and vaginal burning sensation;The compliance rate in the chronic cervicitis with pathological examination before and after operation was 96.23%,the cervical intraepithelial neoplasia in CIN Ⅰ-Ⅱ was 100%,in the CIN Ⅲ was 90.00%.There was no recurrence after followed up for 6 months,nor postoperative infection,bleeding and cervical port adhesion,complications such as cervical stenosis.Conclusion LEEP is simple,rapid,has less complications,shorter operative time,less bleeding,less pain in patients with advantages,such as safe and effective treatment of cervical disease,is an ideal treatment method for cervical lesions,and do not require hospitalization,reducing the cost of treatment and worthy to application.
