老年高血压病患者动态血压与颈动脉粥样硬化程度及左室肥厚的关系

目的探讨老年高血压病患者动态血压参数对颈动脉内一中膜厚度及左室肥厚的影响。方法老年高血压病患者95例经颈动脉超声和超声心动图检查被分为颈动脉内膜增厚组(n=67例)和内膜正常组(n=28例);左室肥厚组(n=30例)和左室正常组(n=65例)。测定并比较动态血压参数,血脂、血糖等生化指标。结果 (1)颈动脉内膜增厚组和内膜正常组的临床指标比较差异无统计学意义(P>0.05);(2)颈动脉内膜增厚组和内膜正常组动态血压参数比较差异有统计学意义(P<0.05);(3)左室肥厚组和左室正常组的临床指标比较差异无统计学意义(P>0.05);(4)左室肥厚组和左室正常组的动态血压参数比较差异有统计学意义(P<0.05)。结论高血压病动态血压参数对颈动脉内一中膜增厚和左室肥厚有显著影响,可能参与其发病机制。

Relationship between ambulatory blood pressure and carotid artery intia-media thickness and left ventricular hypertrophy in old patients with hypertension

Objective To investigate the influence of ambulatory blood pressure monitoring（ABPM）parameters on carotid intima-media thickness（IMT）and left ventricular hypertrophy（LVH）in the old patients with hypertension.Methods The 95 old hypertension patients underwent carotid arteries ultrasound examinations and echocardiography and were divided into increased IMT group（n=67）,normal IMT group（n=28）,LVH group（n=30）and normal left ventricle group（n=65）.The ABPM examination were performed in all the patients.Results（1）There were no significant differences in clinical manifestations between the IMT increase and normal group.（2）The parameters of ABPM in IMT increase group were higher than those of normal group（P〈0.05）.（3）There were no significant differences in clinical manifestations between the LVH group and normal group.（4）The parameters of ABPM in LVH group were higher than those in normal group,which were significantly different（P〈0.05）.Conclusion There is a significant influence of ABPM parameters on IMT of carotid artery and LVH.ABPM parameters abnormality may be involved in the onset of LVH and IMT.
