摘    要:目的探讨眼底动脉硬化与颅内动脉硬化血流动力学改变的相关性。方法对433例常规体格检查提示眼底动脉硬化患者(A组),根据有或无高血压、糖尿病及高血脂病史者,分为A1组(307例)和A2组(126例),将同期204例无眼底动脉硬化及心脑血管病史者作为对照组(B组),采用经颅多普勒超声检测颅内动脉血流动力学的变化及血流频谱特征,比较两组对象血流频谱Ⅰ型(正常血流频谱)、Ⅱ型(高阻力型)、Ⅲ型(一般硬化型)、Ⅳ型(血管狭窄型,狭窄率>50%)的检出率,分析眼底动脉硬化与颅内动脉硬化,高血压、糖尿病、高血脂与眼底动脉硬化及颅内动脉硬化的相关性。结果 A组患者Ⅰ型血流频谱检出率明显低于B组,分别为35.1%与66.6%(P

Abstract:Objective To analysis the relationship between the hemodynamic change of eyeground artherosclerosis (EGAS) and that of intracranial arteriosclerosis (ICAS). Methods 433 patients with EGAS were divided into two groups (A1, A2) according to their history of hypertension (HP), diabetes metus (DM) and hy-perlipoemia (HC). Group B were 204 patients without eyeground, coronary and cerebral vascular disease. TCD was used to find intracranial hemodynamic change and character of blood wave. Check-out rate (COR) of different type of blood wave was contrasted between group A and group B. The relationship between EGAS and ICAS, the relationship between EGAS, ICAS and HP, DM, HC were analyses. Results The COR of type I wave in group A patients (35. 1% ) was higher than that in group B patients (66. 6% ). The COR of other type wave in group A patients was lower than that in group B patients. The abnormal hemodynamic COR of group Al patients (68. 7% ) was higher than that of group A2 (55. 6% ). EGAS was related to ICAS. ICAS was related to HP, DM and HC. There were 33. 3 percent of patients with EGAS whose TCD examination showed normal blood wave of intracranial vessels. Conclusion EGAS is related to ICAS. The patients without EGAS can not exclude ICAS. TCD can find the hemodynamic change of ICAS at early stage.
