摘    要:目的应用斑点追踪技术测得解旋率(UntwR)与组织多普勒技术(DTI)评价不同年龄段正常人左心室的舒张功能,并探讨两者的相关性。方法健康体检者94例,按年龄分为青年组(18~45岁)、中年组(46~64岁)和老年组(65~79岁)及高龄组(≥80岁)。取胸骨旁左心室心尖和心底短轴切面二维灰阶图像,用EchoPAC软件行脱机分析并计算UntwR;应用脉冲多普勒测定二尖瓣口血流早期充盈峰峰值速度(E),应用DTI技术测定二尖瓣环室间隔侧舒张早期峰值速度e’,计算两者比值E/e’。结果随年龄增加,UntwR测值逐渐减低,4组间比较差异均有统计学意义(P均0.05);UntwR与E/e’及e’相关系数r分别是0.54,0.53(P均0.01)。结论 UntwR与目前常用的评价左心室舒张功能的多普勒指标相关性良好,UntwR可无创评价由年龄引起的正常人左心室舒张功能的变化。

Abstract:Objective To determine if the degree of untwisting rate (UntwR) is related to estab- lished parameters of diastolic function. Methods Ninety-four healthy subjects were classified into four groups by age, a group of 19-45 years old,a group of 46-64 years old, a group of 65-79 years old and a group of I〉80 years old. Basal and apical left ventricular(LV) short-axis images were achieved in the total population. The peak early (E) transmittal filling velocities, mitral septal annular tissue Doppler velocity ( e' ) were measured. The ratio of E/e' and UntwR were calculated. Results Along with an increase of ages, UntwR decreased significantly ( P 〈 0.05 ) and the difference had no significant difference among the 4 groups (P 〉 0.05). Both E/e' and e' were correlated with UntwR ( P 〈 0.01 for both). Conclusions The UntwR is related to the established echocardiographic measures of diastolic function and the age-related changes of left ventricular diastolic function could be evaluated by non-invasive speckle tracking imaging(STI).
