奎硫平与利培酮治疗老年精神分裂症的对照研究

目的比较奎硫平和利培酮治疗老年精神分裂症的疗效及不良反应，探讨奎硫平在老年精神分裂症临床治疗的应用价值。方法将96例符合CCMD-3的老年精神分裂症患者随机分为两组，分别给予奎硫平与利培酮治疗，共8周。分别在治疗前及治疗后第2、4、6、8周末，采用阳性症状与阴性症状量表（PANSS）评定疗效，不良反应症状量表（TESS）评定不良反应。结果两组患者经8周治疗后，其有效率分别为91．7％和89．6％，两种药物疗效无显著性差异（P〉0．05）。奎硫平组的不良反应发生率低于利培酮组。利培酮组锥体外系反应的发生率、体重增加和内分泌改变显著高于奎硫平组（P〈0．05）。结论两种药物对治疗老年精神分裂症均有确切疗效，且安全性高，但二者的不良反应的差异表明奎硫平比利培酮对治疗老年精神分裂症病人效果更好。

Comparative study of efficacy and safety of Quetiapine and Risperidone in the treatment of elderly schizophrenia

Objective By comparing the efficacy and adverse reactions of Quetiapine and Risperidone in the treatment of elderly schizophrenia, to argue the clinical worthiness of Quetiapine in treating elderly schizophrenia. Methods 96 patients with schizophrenia by CCMD - Ⅲ were randomly divided into two groups treated with Quetiapine and Risperidone for 8 weeks. The positive and negative syndrom scale （PANSS） and treatment emergent symptoms scale （TESS） were used to evaluate effects and adverse reaction before and at the ends of 2,4, 6,8 weeks of treatment. Results The total effective rates of Quetiapine and Risperidone groups were 91.7 %and 89.6% ,respectively after 8 weeks. There wvas no significant difference between the two groups （P〉0.05）. Incidence of adverse effects was lower in Quetiapine group. The rates of extrapyramidal symptoms, weight gain and endocrine change were significantly higher in Risperidone group than those in Quetiapine group （P〈0.05）. Conclusion Two drugs on the treatment of elderly schizophrenia have the exact effect and safety. However Quetiapine have better effect in treating elderly schizophrenia than Risperidone, considering less adverse reactions made by Quetiapine.
