化疗联合适形放疗治疗局晚期胰腺癌的临床分析

目的　评价化疗结合三维适形放射治疗晚期胰腺癌的疗效。方法　选自 1999- 0 9～ 2 0 0 3- 0 3收治的 4 9例晚期胰腺癌患者。其中 ,采用化疗结合三维适形放射治疗 (综合组 ) 2 3例 ,单独应用化疗治疗 (对照组 ) 2 6例。结果　 2组治疗过程中均有不同程度恶心、呕吐和白细胞、血小板减少 ,对症治疗后缓解。疼痛缓解率综合组和对照组分别为 78.2 %和 76 .9%。综合组总有效率 (CR +PR) 6 9.6 % ,对照组总有效率 (CR +PR) 30 .8% ,2组近期疗效差异有极显著性意义 (P <0 .0 1)。综合组和对照组的 6个月和 12个月年生存率分别为 6 9.6 % (16 / 2 3)、34.7% (8/ 2 3)和 38.4 % (10 / 2 6 )、7.7% (2 / 2 6 ) ,均有显著性意义 (P <0 .0 5 )。结论　化疗与三维适形放射治疗结合 ,在提高局部控制率、延长生存期方面优于单纯化疗。

Chemotherapy with Three Dimensional Conformal Radiotherapy for Locally Advanced Pancreatic Cancer

Objective To evaluate the effect of the chemotherapy with three dimensional conformal radiotherapy on the locally acvanced pancreatic cancer. Methods 49 patients with advanced pancreatic cancer from September 1999 to March 2003, the therapy group 23 patients were treated with three dimensional conformal radiotherapy and control group 26 patients were treated with chemotherapy single. Results Pain relief was 78.2% in therapy group and 76.9% in control group. The general remission rate (CR PR) was 69.6% in therapy group and 30.8% in control group with significant difference between two groups (P<0.01). The survival rates of 6 months and 12 months respectively 69.6%, 34.7% and 39.4% and 7.7% in therapy group and control group with significant difference. Conclusion Chemotherapy associated with three dimensional conformal radiotherapy on the locally advanced pancreatic cancer might control and elongate lifetime of patients which is better than single chemotherapy.
