奎的平与氯丙嗪治疗精神分裂症患者的疗效及改善生活质量的比较

目的比较奎的平与氯丙嗪治疗精神分裂症患者的疗效及改善生活质量的效果。方法将精神分裂症患者分为奎的平或氯丙嗪治疗组。观察3个月后，采用阳性症状与阴性症状量表(PANSS)评定疗效，采用副反应量表(TESS)评定副反应，以生活质量综合问卷(GQOL-74)评定生活质量。结果两组患者治疗前后PANSS总分及各因子分比较均无显著性差异；奎的平的过度镇静作用，抗胆碱能副作用较少，几乎不引起EPS和催乳素的升高；两组患者治疗3个月后心理功能维度、社会功能维度、总维度分均显著高于治疗前；而且奎的平组包含躯体功能维度和以上3种维度还显著高于氯丙嗪组。结论奎的平副作用较少，对提高患者生活质量的效果明显优于氯丙嗪，有利于患者重返社会后适应生活。

Comparative study between the quality of life and its curative effect in schizophrenic patients treated with quetiapine or chlorpromazine

Objective To compare the life quality and psychiatric symptoms of two groups of schizophrenic patients who were treated with quetiapine or chlorpromazine respectively. Methods Schizophrenic patients were randomly divided into two groups receiving quetiapine or chlorpromazine respectively and were followed up for 3 months, they were evaluated with GQOL1-74 about their life qualities and were assessed with positive and negative symptom scale(PANSS) about curative effects before and after 3 months,and TESS was used to evaluated the side effects.Results Curative effects of two groups were similar.The general scores and gene scores of PANSS were not markedly difference.Quetiapine has less excess calmness and less anticholinergy and hardly bring EPS and hoist prolactin.After 3 months of treatment,two groups gained the scores of mental function dimensions,social function dimensions and total dimentions which were significantly higher than that before treatment.Otherwise,the scores of quetiapine in physical function dimensions including three kinds of dimensions upward were higher than that of chlorpromazine.Conclusion Quetiapine has less side-effect and superior to chlorpromazine in the life quality for schizophrenics quetiapine is favorable for schizophrenic patients to recovery and return to society.
