小脑血管母细胞瘤的磁共振诊断

目的探讨小脑血管母细胞瘤的MRI表现,提高对该病的诊断水平。方法回顾性分析经手术病理证实的30例小脑血管母细胞瘤的MRI表现。结果右侧小脑半球15例,左侧小脑半球10例,小脑蚓部6例。大囊小结节型21例和单纯囊型2例的囊壁光滑,囊性部分T1WI表现为稍高于脑脊液的低信号,T2WI表现为高或稍高信号,FLAIR呈等或稍信号,增强扫描壁结节明显强化,单纯囊型囊壁亦未见强化。实质型8例T1WI呈稍低信号,T2WI呈高信号,增强扫描显著强化。本组共12例出现瘤周流空血管影,共14例出现梗阻性脑积水。结论大部分小脑血管母细胞瘤MRI表现具有一定的特征性,少部分具有其它多样性表现。

MR Diagnosis of Hemangioblastoma

Objective To investigate the imaging features of cerebellar hemangioblastoma.Methods MRI data of 30 cases of cerebellar hemangioblastoma proved by surgery and pathology were analyzed retrospectively.Results Lesions of 15 cases located in right cerebellar hemisphere,10 cases in left and 6 cases located in vermis.21 cases showed typical large cyst with small nodule and 2 cases showed single simple cyst.The cyst had hypointense on TlWI and hyperintense on T2WI.The nodule was enhanced markedly with contrast materials.There was no enhancement of the wall after the administration of the contrast agents in pattern of simple cyst.8 cases appeared solid hemangioblastoma.The mass of this pattern,which was apparently enhanced,had slightly low signal on TlWI and slightly high signal on T2WI.Flow void signal of blood vessels was revealed in 12 cases.Obstructive hydrocephalus was revealed in 14 cases.Conclusion MRI findings of most cerebellar hemangioblastoma have some characteristics.It is important to be familiarized with the characteristics for the diagnosis and differential diagnosis.
