尿激酶在锥颅碎吸治疗高血压性脑出血中的剂量与疗效分析

【目的】研究比较锥颅碎吸治疗高血压性脑出血中运用不同剂量的尿激酶溶解和冲洗血肿的疗效。【方法】将48例高血压性脑出血的患者随机分为A、B、C三组。A组运用尿激酶1万单位（14例），B组运用尿激酶4万单位（20例），C组运用尿激酶10万单位（14例）进行溶解和冲洗血肿。治疗前后进行评分比较。【结果】A、C治疗组问总有效率存在显著性差异（P〈0．05），B、C治疗组间有显著性差异（P〈0．05），而A、B组间无显著性差异（P〉0．05）。【结论】运用小剂量尿激酶溶解和冲洗血肿较大剂量效果好，可能与尿激酶引起再出血有关。

Dose-response Analysis of Urokinase in Hypertension-induced Cerebral Hemorrhage Treated with Stereotaxic Aspiration

Objective] To compare the therapeutic effect of different dosage of urokinase applied in dissolving and douching haematoma while stereotaxic aspiration was used in treating hypertension-induced cerebral hemorrhage. Methods]Fifty-eight hypertension-induced cerebral hemorrhage patients were divided into three groups. To dissolve and douche haematoma, ten thousand unit of urokinase was used in group A （ n = 14）. Forty thousand unit of urokinase was used in group B （ n =20）. One hundred thousand unit of urokinase was used in group C （ n =14）. Clinical neurologic impairment degree score of cerebral apoplexy were compared before and after treatment. Results] There was significant difference in the total therapeutic effect between group A and group C （ P 〈0.05）. There was also significant difference in the total therapeutic effect between group B and group C （ P〈0. 05）. And there was no significant difference in the total therapeutic effect between group A and group B （ P〉0.05）. Conclusion] Low dose of urokinase has better therapeutic effect in dissolving and douching haematoma than large dose of urokinase. Maybe this is concerned with its rehaemorrhagia risk.
