摘    要:目的: 应用血管内超声(intravascular ultrasound,IVUS)技术探讨血清高敏性C反应蛋白(hsCRP)与冠心病不稳定型心绞痛(UAP)的关系。方法: 38例拟诊为冠心病患者行冠状动脉造影(coronary angiography,CAG)和IVUS检查,其中8例患者经CAG和IVUS检查未发现冠脉有明显病变为对照组;30例经IVUS检查发现冠脉有明显狭窄病变,为冠心病组,其中UAP 18例、稳定型心绞痛(SAP)12例。检测所有患者的hsCRP浓度。结果: 38例患者均有不同程度的内膜增厚;8例对照组无动脉粥样硬化(AS)斑块;30例冠心病患者IVUS示有不同类型的AS斑块。UAP组患者血清hsCRP水平显著高于SAP组及对照组;不稳定斑块组hsCRP水平高于稳定斑块组。结论: UAP患者血清hsCRP水平明显增高,提示hsCRP水平升高与AS斑块不稳定有关。

Intravascular ultrasound evaluation of high-sensitivity C-reactive protein in unstable angina patients
Abstract:AIM: To evaluate high-sensitivity C-reactive protein (hsCRP) in unstable angina patients using intravascular ultrasound (IVUS). METHODS: Thirty-eight patients suspected of having coronary heart disease (CHD) underwent coronary angiography and IVUS examinations. Eight patients without significant coronary artery stenosis were assigned to the control group and 30 patients with significant coronary artery stenosis were assigned to the CHD group including unstable angina pectoris (UAP, 18 cases) and stable angina group (SAP, 12 cases). Serum hsCRP concentrations were detected in all patients. RESULTS: Endothelial thickening of varying degrees was observed in all 38 patients. IVUS showed atherosclerotic plaques of different types in the 30 patients in the CHD group but no atherosclerotic plaques in the control group. hsCRP in UAP cases was significantly higher than in SAP patients and in control group. Levels of hsCRP in unstable plaques were higher than those in stable plaques. CONCLUSION: Serum hsCRP is significantly higher in patients with UAP, indicating that increasing levels of hsCRP are correlated with unstable plaques of UAP patients.
