基于文献计量分析的我国医联体发展动阻力研究

目的:确定医联体发展模式中共性的动力和阻力,进一步增强发展动力、减弱发展阻力,从而为促进医联体良性发展提供参考。方法:以“CNKI”和万方数据库为检索对象,以“医联体”或“医疗联合体”或“医共体”为主题词,最终检索得到文献3173篇,并在此基础上对筛选后的247篇文献进行计量分析。结果:医联体的文献数量从2013年开始逐年增加;文献研究者层次较高;文献研究的问题地域范围广泛,全国范围的问题占53.0%;政策推进和问题导向是医联体发展的主要推动力,其中国家出台的系列政策和指导意见推动作用最为突出;患者传统的就诊习惯难改变以及配套政策不健全是医联体发展的主要阻力。结论:国家将医联体建设作为分级诊疗制度建设的重要抓手,出台卫生政策和指导意见推动医联体发展;当前医联体发展阻碍力较多且关键阻碍力作用显著,其中患者认可度低是限制发展最大的阻碍力。应进一步完善制度采取措施减弱阻力增强动力。

Bibliometric Analysis of Dynamic Resistance in The Development of Medical Services Alliance in China

Objective To determine the power and resistance of medical services alliance development,and provide references for promoting its development.Methods Chinese Journal Full-text Database(CNKI)and Wanfang database were used to retrieve the literature,with the subject term of“medical services alliance”or“medical association”.Based on which,quantitative analysis was used.Results The number of related literature kept increasing year by year from 2013.The level of literature researchers was high.And the regional range of literature was wide,47.4%of which was nationwide.The main development impetus was medical reform policy and problem-based idea,especially from the national policies.The main resistance was patients*unchangeable medical-seeking habit and imperfect supporting policy.Conclusion It is suggested for the government to value the construction of medical services alliance in the construction of hierarchical diagnosis and treatment system.There had many obstacles in its development,and the key hindrance is significant,in which the greatest resistance was the patients'poor cognition.It is necessary to further improve the system to weaken the resistance and strengthen the impetus.
