二维及彩色多普勒超声诊断肺动脉口狭窄

摘    要:目的:提高二维及多普勒超声对肺动脉口狭窄的诊断正确率,降低漏诊率。方法:对102例经手术证实的肺动脉口狭窄病例进行回顾及其二维及多普勒超声表现,并将超声检查的狭窄部位及大小与手术结果进行对比。结果:184处狭窄中超声正确诊断161处,准确率87%。多部位狭窄多见,102例中81例为多部位狭窄,除1例外101例均合并其它内畸形。超声测得的狭窄孔径与手术测值无显著性差异。结论:单一部位狭窄可依据彩色及脉冲普勒确定狭窄部位,连续多普勒判定狭窄程度;多部位狭窄多普勒难以确定血流加速部位,需结合二维图像寻找狭窄;合并其它心内畸形时需要通过多个非标准切面更好显示狭窄病变。

Diagnosis of Pulmonic Stenosis Using Two-dimentional and Color Doppler Ultrasound
Abstract:Objective:To improve the diagnostic rate of pulmonic stenosis using two dimensional and Doppler echocardiography and to reduce the rate of misdiagnosis.Methods:One hundred and two cases with pulmonic stenosis confirmed by operation were studied.Their characteristics of two dimensional and Doppler echocardiography were analysed.The results of ultrasonic examination were compared with those of operation.Results:One hundred and sixty one stenosis sites were detected correctly among 184 stensis sites,with the rate of diagnosis 87%.Multifocal stenosis predominated with 81 among 102 cases and all but one were complicated with other cardiac anomalies.There were no significant differences between the diameters of stenotic orifice measured in echocardiography and those in operation.Conclusions:For single stenosis,we can focal the trouble site using spectra and color Doppler ultrasound.As to the multifocal stenosis,we must search carefully in two dimensional imaging because of the difficulties to point to the blood accelerating parts with Doppler ultrasound.When complicated with other cardial anomalies,it needs many unstandard sections to give a better show for trouble sites.
