子宫动脉栓塞术治疗子宫腺肌病的不良反应与并发症分析

目的探讨子宫动脉栓塞术（TUAE）治疗子宫腺肌病疗效及不良反应与并发症的防治措施。方法66例经临床和B超、腹腔镜检查诊断为子宫腺肌病者，经导管子宫动脉注入超液化碘油、PVA，或平阳霉素、明胶海绵颗粒栓塞子宫动脉，栓塞后1、3、6、12、18、24个月定期复查观察疗效及不良反应与并发症发生情况。结果66例子宫腺肌病TUAE术有效率90％，TUAE栓塞后综合征、膀胱刺激症6例（9．09％），胃肠道反应10例（15．15％），阴道少量流血9例（13．63％），宫颈部缺血性坏死3例（4．54％），小阴唇溃疡1例（1．51％）。TUAE的不良反应和并发症经对症或特殊处理恢复正常。结论子宫动脉栓塞术治疗子宫腺肌病，近期疗效可观，术中术后不良反应和并发症是可防治的。

Assay of Side Reaction and Complication In Uterine Arery Embolization for Treatment of Abenomysis

Objective To assess the therapeutic effectiveness of uterine artery embolization(UAE)for the treatment of abenomysis and the prevention of side reaction and complication.Methods UAE procedures were performed in 66 patients with adenomyosis.The diagnosis of adenomyosis was besed on establish Ultrasound and clinical symptoms such as enuneniopathy,uterine enlargement and heavy menstrual bleeding.A Cordis cobra catheter were placed into uterine arteries and ultra-fluid lipiodol mixed with bleomycin,and fresh gelfoam were used to embolise the arteries.After treatment the therapeutic effectiveness and side reaction and complication.All the patients were investigated respectively after 1,3、6、12,18,24 months.Results The therapeutic effectiveness in the near future of the 66 patients with adenomyosis in this study was considerable.The syndrome after embolization,such as the diseases of urinary bladder were 6 patients(9.09%),the complication of gastrointestinal tract occurred were 10 patients(15.15%),and a bleed in vagina occurred were 9 patients(13.63%),in addition,the ischemic necrosis of cervix were 3 patients(4.54%),the ulser in nympha only occurred were 1 patient(1.51%).In our study,the side reaction and complication.Could be disappeared after especially treatment.Conclusion UAE for the treatment of adenomyosis is an effective and safe method in the near future.The side reaction can be reversible,and the compilation can be prevented.
