B型钠尿肽对早期舒张性心力衰竭的诊断价值

目的探讨血浆B型钠尿肽（BNP）对早期舒张性心力衰竭（DHF）的诊断价值。方法收集170例DHF患者为观察组，超声心动图检查左心室射血分数（LVEF）〉150％，符合DHF诊断标准，采用NYHA心功能分级法将观察组分为心功能Ⅱ级、Ⅲ级和Ⅳ级；收集102例有同样基础疾病无临床心力衰竭症状体征者为对照组，两组均检测血浆BNP水平，进行组内和组间比较。结果观察组血浆BNP水平显著高于对照组（P〈0．01），并随NYHA心功能级别增高而升高（P〈0．01）。结论血浆BNP水平升高可作为早期DHF的诊断指标和病情评估。

Significance of blood BNP in early diagnosis of diastolic heart failure

Objective To investigate the clinical significance of plasma B-type natriuretic peptide （BNP） in early diagnosis of diastolic heart failure （DHF）. Methods All of 170 cases of DHF patients who were in line with the provi sions of the diagnostic criteria with echoeardiography left ventrieular ejection fraction （LVEF） ≥50% as observe group, According to NYHA grading method of HF, the patients of Observation Group were rated into three ranks as G-II, G- III, and G-IV.102 patients with same underlying disease butno .clinical symptoms of heart failure as control group. Plasma levels of BNP were measured and set within and between group comparison. Results Plasma levels of BNP in observe group was obviously higher than in control group （P 〈 0.01 ） and was elevated with the level of cardiac function NYHA rises （P 〈 0.01）. Conclusion Plasma level of BNP is available as diagnostic indicators and condition assessment of early diagnosis of diastolic heart failure （DHF）.
