新生儿黄疸胆红素和总胆汁酸测定的临床意义

摘    要:目的探讨黄疸胆红素和总胆汁酸测定在新生儿黄疸中的临床意义。方法选取2011年1月~2012年1月我院小儿科收治的110例新生儿黄疸,根据临床诊断将110例新生儿黄疸患儿分为生理性黄疸组(n=41)与病理性黄疸组(n=69)两组,再选取正常的新生儿90例为对照组。对三组新生儿同时予以胆红素和总胆汁酸测定.并统计分析。结果生理性黄疸组的IBIL、TBIL水平与对照组相比,差异有统计学意义(t=46.845,47.162,P〈0.01);而生理性黄疸组的TBA水平与对照组相比,差异无统计学意义(f=1.764,P〉0.05)。病理性黄疸组的TBA、IBIL、TBIL水平与对照组相比,差异有统计学意义(t=18.931,74.897,73.466,P〈0.01)。病理性黄疸组TBA水平与DBIL、ALT、AST、ALP及GGT各项肝功能指标有直线相关关系。结论对新生儿黄疸采用胆红素检测的同时予以总胆汁酸检测,可利于临床对黄疸类型、黄疸程度、胆汁淤积程度及肝细胞损伤等情况的判断,从而利于新生儿黄疸的早期诊治及预后,确保新生儿的健康。

Clinical significance of jaundice bilirubin and total bile acid determination in neonate

Abstract:Objective To study the determination of jaundice bilirubin and total bile acid in the clinical significance of newborn Huang Danzhong. Methods From 2011 January to 2012 January in our hospital pediatric treated 110 cases of neonatal jaundice, according to the clinical diagnosis of 110 cases of neonatal jaundice were divided into physiological jaundice group (n = 41 ) and pathologic jaundice group (n = 69 ) in normal newborn group, 90 cases in the control group. On three groups of infants the bilirubin and total bile acid were tested at the same time , and did statistical analysis. Results IBIL, TBIL levels as compared with the control group physiological jaundice group, had significant difference ( t=46.845, 47.162, P 〈 0.01 ), physiological jaundice group TBA levels compared with the control group, had no significant difference (t = 1.764, P 〉 0.05 ). Pathologic jaundice group TBA, IBIL, TBIL levels as compared with the control group, had significant difference (t = 18.931, 74.897, 73.466, P 〈 0.01). A linear correlation between the pathological jaundice group TBA level and DBIL, ALT, AST, ALP and GGT liver function indexes. Conclusion For neonatal jaundice with bilirnbin detection at the same time be the total bile acid detection, can be beneficial to clinical types of jaundice, jaundice, cholestasis and liver cell damage, judgment, which is beneficial to early diagnosis and treatment and prognosis of neonatal jaundice, ensure that the newborn health.
