13例中耳胆固醇肉芽肿手术治疗的回顾性分析

目的探讨中耳胆固醇肉芽肿不同手术治疗方案的选择及其治疗结果。方法采用回顾性研究，对我院2009～2012年手术治疗的13例中耳胆固醇肉芽肿患者的临床资料进行总结分析。结果4例患者采用鼓室探查＋鼓膜切开置管术，其中1例术后半年复发，二次手术采用开放式鼓室成形术，6例采用完壁式乳突切除＋面隐窝开放＋鼓膜切开置管术，2例采用开放式乳突切除＋鼓室成形术，1例采用乳突根治术，术后随访1年以上均无复发。结论中耳胆固醇肉芽肿治疗应当彻底清除病变，重建通气引流通道，术前充分评估病变范围，选择适宜的手术方式。

Retrospective study of surgical treatment of middle-ear cholesterol granuloma in 13 cases

Objective To investigate different surgical treatments of middle-ear Cholesterol granuloma and their outcomes. Methods All 13 patients with middle-ear cholesterol granuloma were retrospectively study, all of them were received surgical treatment during 2009 to 2012 in our department. Results 4 cases were treated with tympanic tube insertion, one of them was recurred after six months, and then canal wall down mastoidectomy was applied. 6 cases were received canal wall up mastoidectomy and tympanic tube insertion, and the facial recess was opened at the same time. 2 eases was applied with canal wall down mastoidectomy,and a PORP was used for ossicularplasty. Only one case with Sensorineuralhearing loss was treated with radical mastoidectomy. All of them was followed up more than one year without recurrence. Conclusion The treatment of middle-ear cholesterol granuloma should compeletly remove the lesion and reconstruct adequately draining channel. Preoperative assessment should be done thoroughly for a proper treatment option.
