小切口白内障囊外摘除术治疗高度近视合并硬核白内障疗效观察

摘    要:目的观察小切口白内障囊外摘除术治疗高度近视合并硬核白内障的临床疗效。方法自2003年7月~2012年12月,我院眼科对眼轴≥26mm的高度近视合并Ⅲ级以上硬核白内障患者156例(168只眼)行小切口白内障囊外摘除术。结果术后3个月,161只眼(96%)术后矫正视力均有不同程度提高,7只眼(4%)因合并严重视网膜脉络膜萎缩病灶故视力无提高。术中并发症:后囊膜破裂5只眼(3.0%)。术后并发症:17只眼角膜可逆性内皮水肿(10.1%);8只眼前房有渗出反应(4.8%);5只眼前房积血(3.0%);22只眼后囊膜混浊(13.1%);2只眼术后视网膜脱离(1.2%)。结论小切口白内障囊外摘除术是治疗高度近视合并硬核白内障较理想的手术方式.患者视力恢复良好、并发症少。

Clinical effect of small-incision extracapsular cataract extraction on high myopic eyes with hard nucleus cataracts

Abstract:Objective To observe the clinical effects on small-incision extracapsular cataract extraction in high myopic eyes with hard nucleus cataracts. Methods From July 2003 to December 2012, the cataract operation was performed on 156 patients (168 eyes) with high myopic eyes, whose axial length beyond 26 mm complicated with above grade Ⅲ hard nucleus cataracts. Results At postoperative 3 months, the corrected visual acuity was improved in different degree 161 eyes (96%), the corrected visual acuity was not improved complicated with severe retinal choroid in 7 eyes (4%). In the major complications, posterior capsule rupture occurred in 5 eyes (3.0%),cornea edema inl7 eyes (10.1%), uveitic reaction in 8eyes (4.8%), hyphema in 5 eyes (3.0%). Posterior capsular opacity in 22 eyes (13.1%), retinal datachment in 2 eyes (1.2%). Conclusion Small-incision extracapsular cataract extraction is a good operative method in high myopic eyes with hard nucleus cataracts ,which the vision was improved obviously and the less complication .
