甲状腺激素替代治疗对老年甲状腺功能减退患者血脂及心功能的影响

目的分析甲状腺激素替代治疗对老年甲状腺功能减退患者血脂及心功能的影响。方法取本院内分泌科于2008年9月～2012年9月间门诊及住院收治的老年甲状腺功能减退患者176例作为研究对象，所有患者均给予甲状腺激素替代治疗，比较患者接受治疗前后的甲状腺功能指标、血脂相关指标、心功能指标水平。结果所有患者接受治疗后的平均TSH值明显低于治疗前水平，FT3及FT4平均值均明显高于治疗前（P〈0．05）；所有患者接受治疗后的血脂指标平均值均明显低于治疗前（P〈0．05）；接受治疗后患者的心功能明显改善，治疗后的左室射血分数及左室舒张功能值均明显高于治疗前（P〈0．05）。结论使用甲状腺激素替代治疗老年甲状腺功能减退，在有效增强甲状腺功能的同时，可以明显改善患者的血脂水平及心功能。

Influnce of thyroid hormone replacement therapy on lipid and heart function of elderly patients with hypothyroidism

Objective To analyze the effect of decreased blood thyroid hormone replacement therapy and cardiac function in elderly patients with thyroid function. Methods All of 176 cases of elderly patients with hypothyroidism from September 2008 to September 2012 were treated with thyroid hormone replacement therapy, compared with patients before and after treatment, thyroid function index, serum lipids, heart function index difference. Results All the patients after treatment the average vlue of TSH was obviously lower than that before treatment, FT3 and FT4 were significantly higher than average value before treatment （P 〈 0.05）. After treatment, blood lipid index average values were significantly lower than those before treatment （P 〈 0.05）, heart function of patients after treatment significantly improved after treatment, left ventricular the ejection fraction and left ventricular diastolic function were signifieantly higher than those before treatment （P 〈 0.05）. Conclusion The use of thyroid hormone replacement therapy in elderly hypothyroidism, effectively enhance the thyroid function at the same time, can significantly improve the blood lipid level and heart function in patients.
