心电图SavR诊断高血压左心室肥大（LVH）的临床价值

目的探讨心电图SavR电压在诊断高血压左心室肥大（LVH）中的临床价值。方法经超声心动图检查诊断为左心室肥大的高血压患者75例为观察组及健康体检无左心室肥大的正常人120例为对照组．测量各相关R波、S波电压，计算各肢体导联及胸导联Rv5＋Svl并比较SavR、Rv5＋Svl及两者联用诊断左心室肥大的敏感性、特异性及准确性。结果各肢体导联中SavR敏感性（45.3％）较高（P〈0．05），特异性及准确性相近。胸导联Rv5＋Svl敏感性（60．O％）较SavR高，但特异性（85．8％）较SavR（98.3％）低，SavR、Rv5＋Svl准确性为77．9％、75．8％。两者联用时诊断左心室肥大的敏感性和准确性显著提高（分别为88．0％、85．6％）。结论SavR标准是诊断左心室肥大较好指标，与Rv5＋Svl联用可提高高血压左心室肥大诊断的敏感性和准确性。

Clinical value of ECG SavR in diagnosis of hypertensive left ventricular hypertrophy

Objective To investigate the clinical value of ECG SavR voltage in the diagnosis of hypertensive left ven- tricular hypertrophy （LVH）. Methods Seventy-five hypertension patients diagnosed with LVH by untrasounic cardiogram were selected as the observation group and 120 normal people without LVH were selected as the control group. Rele- vant R-wave and S-wave voltage was measured and Rv5＋Svl of each limb leads and chest leads were calculated. The SavR, Rv5＋Svl and the sensitivity, specificity and accuracy of combined application were compared. Results Of each limb leads, the sensitivity of SavR （45.3%） was high （P 〈 0.05） and the specificity and accuracy were similar. The sen- sitivity of chest leads Rv5＋Svl was 60.0%, which was higher than SavR, but the specificity （85.8%） was lower than that of SavR （98.3%）; The accuracy of SavR and Rv5＋Svl were 77.9% and 75.8%. In the diagnosis of LVH, the sensitivity and accuracy of combined application improved significantly （88.0% and 85.6% respectively）. Conclusion SavR is a good indicator in the diagnosis of LVH and its combination with Rv5＋Svl can improve the sensitivity and accuracy of the diagnosis of hypertensive LVH.
