摘    要:目的探讨基因重组生长激素(rHGH)在重症COPD患者治疗中的应用效果。方法选择50例因呼吸衰竭在ICU中行机械通气的COPD患者,随机分成rHGH组和对照组各25例,两组都常规给予抗感染、解痉平喘、营养支持等治疗,rHGH组在上述治疗的基础上加用rHGH。观察两组治疗期间的总蛋白、白蛋白、最大吸气压、自主潮气量的变化,比较两组的平均机械通气时间、脱机成功率及病死率。结果治疗期间rHGH组的总蛋白、白蛋白、最大吸气压及自主潮气量增加明显,和对照组相比差异有统计学意义(P〈0.05),血糖及胰岛素的用量rHGH组比对照组有所增加.但差异无统计学意义(P〉0.05),rHGH组的平均机械通气时间明显缩短,脱机成功率明显增加,病死率降低显著(P〈0.05)。结论重症COPD行机械通气的患者在常规治疗的同时应用rHGH能提高脱机成功率,有效降低死亡率。

Application of recombinant human growth hormone in the treatment of patients with severe COPD

Abstract:Objective To discuss the clinical effect of Human growth hormone recombinant(rHGH) in the treatment of patients with severe COPD. Methods Fifty patients with severe COPD who were given mechanical ventilation due to respiratory failure in ICU department were randomly divided into the rHGH group and the control group,every group was 25 cases. The patients of two groups were given conventional therapy such as anti-infection,relieving spasm and asthma and nutritional support.The patients of rHGH group were added rHGH therapy on the basis of conventional therapy. The total protein,albumin,peak inspiratory pressure,independent tidal volume, average time of mechanical ventilation,off-line success rate and fatality rate in the patients of two groups were observed and compared. Results The total protein,albumin,peak inspiratory pressure,independent tidal volume of the patients in rHGH group increased significantly,and there were significant difference between the rHGH group and the control group (P 〈 0.05). The blood sugar and insulin dosage of the patients in rHGH group increased slightly,bur there were not significant difference between the rHGI-I group and the control group(P 〉 0.05).There were significant difference in average time of mechanical ventilation,off-line success rate and fatality rate between two groups(P 〈 0.05), Conclusion The rHGH in the treatment of patients with severe COPD who were given mechanical ventilation with conventional therapy can improve off-line success rate and lower fatality rate.
