针刺配合康复训练对胫骨平台骨折术后患者关节活动度及应激状态的影响

目的观察针刺配合康复训练对胫骨平台骨折术后患者关节活动度及应激状态的影响。方法选取146例胫骨平台骨折术患者为观察对象,按随机数字表法分为对照组和观察组,每组73例。对照组患者采用康复训练进行术后恢复,观察组在此基础上配合针刺疗法进行治疗,观察并比较两组患者的临床疗效、Lysholm膝关节评分、膝关节活动、疼痛应激指标P物质(SP)、前列腺素E2(PGE2)、神经肽Y(NPY)]与骨折愈合时间。结果治疗后,观察组Lysholm膝关节各项评分显著高于对照组(P<0.05);观察组患者的膝关节活动度,包括伸膝最大角度与屈膝最大角度均显著优于对照组(P<0.05)。观察组3个月内愈合率显著高于对照组(P<0.05)。观察组总有效率显著高于对照组(P<0.05)。治疗7 d后,观察组与对照组的各项疼痛应激指标均高于手术前(P<0.05),但观察组显著低于对照组(P<0.05)。结论针刺配合康复训练能够改善胫骨平台骨折术后患者关节活动度及应激状态,有效缩短患者骨折愈合时间及恢复时间。

Effect of Acupuncture plus Rehabilitation Training on Joint Mobility and Stress State in Patients After Tibial Plateau Fracture Surgery

Objective To observe the effect of acupuncture plus rehabilitation training on joint mobility and stress state in the patients who had undergone tibial plateau fracture surgery.Method One hundred and forty-six patients with tibial plateau fracture surgery were enrolled as subjects and allocated,using a random number table,to control and observation groups,with 73 cases in each group.The control group of patients received rehabilitation training for postoperative rehabilitation and the observation group,acupuncture in addition.The clinical therapeutic effects,the Lysholm knee scores,knee activities,pain stress indicatorssubstance P(SP),prostaglandin E2(PGE2)and neuropeptide Y(NPY)]and fracture healing times were compared between the two groups of patients.Result After treatment,the Lysholm knee item scores,and the knee activities including the maximum knee extension angle and flexion angle were better in the observation group than in the control group(P<0.05).The healing rate during three months and the total efficacy rate were significantly higher in the observation group than in the control group(P<0.05).After seven days of treatment,pain stress indicators were higher in the observation and control groups compared with before surgery(P<0.05)but were significantly lower in the observation group than in the control group(P<0.05).Conclusion Acupuncture plus rehabilitation training improves joint mobility and stress state and effectively shortens fracture healing time and recovery time in the patients after tibial plateau fracture surgery.
