火针点刺治疗肾虚血瘀型不孕的临床研究

目的观察火针点刺对肾虚血瘀型不孕的调经促孕作用及其临床疗效。方法将符合纳入标准的100例肾虚血瘀型不孕患者随机分为火针点刺组(47例)和西药组(53例)。火针点刺组予穴位火针点刺,西药组予口服枸橼酸氯米芬片。监测两组患者的基础体温、子宫内膜、卵泡直径和排卵情况,观察两组治疗前后中医证候评分变化,比较两组临床疗效、妊娠率和不良反应发生率。结果治疗过程中,火针点刺组脱落3例,西药组脱落3例,故最终纳入统计为火针点刺组44例、西药组50例。治疗前,两组中医证候评分、子宫内膜厚度和卵泡直径比较,差异无统计学意义(P>0.05)。治疗后,两组中医证候评分、子宫内膜厚度和卵泡直径均优于治疗前(P<0.05),且火针点刺组治疗后均优于西药组(P<0.05)。火针点刺组临床总有效率和妊娠率均高于西药组,差异均有统计学意义(P<0.05)。两组均未发生不良反应或不良事件。结论火针点刺治疗可有效改善肾虚血瘀型不孕患者的中医证候,改善子宫内膜及促进卵泡发育,临床疗效和妊娠率均高于西药治疗。

Clinical Study on Fire Needle Pricking in Treating Infertility of Kidney Deficiency and Blood Stasis Pattern

Objective To observe the effect of fire needle pricking in regulating menstruation and promoting pregnancy in treatment of infertility of kidney deficiency and blood stasis pattern and its clinical efficacy.Method One hundred eligible patients with infertility of kidney deficiency and blood stasis pattern were randomized into a fire needle pricking group(47 cases)and a Western medication group(53 cases).The fire needle pricking group was intervened by fire needle pricking acupoints,while the Western medication group was intervened by oral administration of Clomifene citrate tablets.The basal body temperature,endometrium,follicle diameter and ovulation were monitored in the two groups,and the changes in the symptoms scores of traditional Chinese medicine(TCM)were also observed.The clinical efficacy,pregnancy rate and adverse reaction rate were compared between the two groups.Result During intervention,there were three dropouts in each of the two groups.As a result,there were 44 cases in the fire needle pricking group and 50 cases in the Western medication group included in the final computing.Before treatment,there were no significant differences in the TCM symptoms scores,endometrial thickness and follicle diameter between the two groups(P>0.05).The TCM symptoms scores,endometrial thickness and follicle diameter were all improved after treatment in both groups(P<0.05),and the TCM symptoms scores,endometrial thickness and follicle diameter in the fire needle pricking group were superior to those in the Western medication group(P<0.05).The clinical total effective rate and pregnancy rate were higher in the fire needle pricking group than in the Western medication group,and the differences were statistically significant(P<0.05).Neither group had adverse reactions or events.Conclusion Fire needle pricking treatment can effectively improve the TCM symptoms,endometrium and follicle development in patients with infertility of kidney deficiency and blood stasis pattern;it can produce more significant clinical efficacy and a higher pregnancy rate than using Western medication.
