经肛门镜下吻合器痔上黏膜环切术与传统痔切除术治疗重度内痔的效果分析

目的比较吻合器痔上黏膜环切术与传统痔切除术的疗效与安全性。方法收集我科经这两种手术方式治疗的患者98例，比较患者术中术后的各项指标，随机分为PPH组和MM组各49例。比较患者术中术后的各项指标。如手术时间、住院时间、创面愈合时间、并发症、肛门功能等。结果PPH组总有效率为97．96％，MM组治疗总有效率为95．92％。PPH组的手术时问、住院时间、创面愈合时间明显低于MM组。PPH组术后并发症发病人数优于MM组，少量出血（出血量〈100mL）、创面疼痛、肛缘水肿、肛门坠胀人数均少于MM组。术后半年对患者进行随访，两组均无肛门失禁发生，PPH组出现肛门狭窄、瘙痒及控便能力下降的患者人数少于MM组，发生肛缘皮赘的人数多于MM组。结论吻合器痔上黏膜环切术在治疗重度内痔方面显著改善了传统痔切除术的众多缺点，治疗重度内痔的并发症和安全性均优于传统切除术，值得临床推广应用。

Effect analysis of transanal endoscopic stapler hemorrhoidectomy versus conventional hemorrhoidectomy in treatment of severe hemorrhoids

Objective To compare of PPH surgery and conventional hemorrhoidectomy efficacy and safety.Methods By the two operation methods in 98 patients,comparing patients with preoperative and postoperative indicators,were randomLy divided into PPH group and MM group with 49 cases in each group.Comparison of patients with preoperative and postop-erative indicators,such as operation time,hospitalization time,wound healing time,complications,such as anal function.Results PPH group’ total effective rate was 97.96%,MM treatment group’ total effective rate was 95.92%.PPH group in the operation time,hospitalization time,wound healing time was significantly lower than that of MM group.The incidence of postoperative complications of PPH group was less than that in MM group,a small amount of bleeding(hemorrhage < 100 mL),wound pain,edema of crissum,anus pendent swells the numbers were less in group MM.Half a year after operation on the patients were followed up,two groups had no anal incontinence,anal stenosis group PPH appears,pruri-tus and bowel control ability to drop in the number of patients less than group MM,occurrence of crissum skin tags were more in number than the MM group.Conclusion Procedure for prolapse and hemorrhoids in the treatment of severe hem-orrhoids has significantly improved the traditional hemorrhoidectomy in treatment of severe hemorrhoids many disadvan-tages,complications and safety were superior to the conventional excision,worthy of clinical application.
