ELISA和TRUST法检测磷脂类抗体在梅毒诊疗中的应用比较

目的 采用ELISA和TRUST法测定梅毒患者血清中磷脂类抗体的浓度和滴度，探讨其在梅毒诊疗中的不同作用。方法 采用梅毒螺旋体特异性抗体明胶凝集试验（TPPA）、TRUST、ELISA法抗心磷脂抗体（ACA）定量法对94例梅毒患者、32例磷脂综合征患者血清进行检测。结果 94例TPPA阳性患者中。TRUST阳性83例，ACA定量（ELISA）阳性48例。TRUST阳性率88．37％，ACA定量（ELISA）阳性率51．06％。32例磷脂综合征患者血清TRUST滴度最高为1：2。其他多数为阴性；ACA均值为235u／ml。结论 （1）ACA定量（ELIsA）作为梅毒初筛试验敏感度低于TRUST试验。（2）ELISA法测得ACA浓度和TRUST滴度与梅毒的病程呈平行关系，TRUST滴度升高表明梅毒螺旋体大量增殖，ELISA法所测ACA升高提示组织细胞破坏增加。（3）在疗效观察中，特别是晚期梅毒患者，ELISA法所测ACA浓度的敏感度高于TRUST滴度。

Comparison of ELISA and TRUST on Detecting the Anticardiolipin Antibody Concentrations During the Diagnosis and Treatment of Syphilis

Objective To Detect the anticardiolipin antibody (ACA) concentrations by ELISA and TRUST in serum of the syphilis suffers,and to discuss the different instructed signification during the diagnosis and treatment in syphilis. Methods The TPPA,TRUST and ACA quantitative analysis techniques were adopted to examine serum samples of 94 syphilouses and 32 phospholipid syndrome suffers.Results Of 94 samples of TPPA positive, there were 83 samples TRUST positive,48 samples ACA quantification positive. TRUST positive ratio was 88.37%, ACA quantification positive ratio was 51.06%; the average of serum ACA of 32 phospholipid syndrome suffers was 235 U/ml. Conclusion (1) As the primary syphilis screening methods, ACA quantification analysis's sensitivity is lower than TRUST's.(2) Though the development of ACA and TRUST concentrations is parallel during the syphilouses sickening period, they show different meanings: the hoist of TRUST account for the proliferate of syphilis leptospira, the hoist of ACA clue to the increase of the breakage of tissue cell . (3) The curative effect observation shows,ACA has the higher sensitivity than TRUST method, especially for the later syphilouses.
