骨科内置物置入手术前后CRP检测的临床意义

目的观察CRP在骨科内置物置入手术前后的变化规律,并探讨其临床意义。方法选择自愿接受CRP及ESR检测的骨科内置物置入手术患者1600例,采用随机数字表法分成A、B组,每组各800例,其中A组患者行术前、术后1、3、5、7、14d及30dCRP指标监测,B组患者进行相同时间点ESR指标监测,并对结果进行统计学分析。结果 A组患者术后第3天CRP指标达到最高值后逐渐回落,术后30dCRP指标降至正常值;术后第3天CRP指标明显高于术前（P〈0.05）;但术前与术后30dCRP指标组间比较,差异无统计学意义（P〉0.05）。B组患者术后第7天ESR指标达到最高值后逐渐回落,术后30dESR指标降至正常;术后第7天ESR指标明显高于术前（P〈0.05）;但术前与术后30dESR指标组间比较,差异无统计学意义（P〉0.05）。结论患者血浆CRP水平检测可为术后抗生素合理使用和早期术后感染的筛选提供可靠依据,具有显著临床价值。

Department of Orthopedics Implant Placement Operation and the Clinical Significance of Detecting CRP

Objective To observe the CRP in the Department of orthopedics implant placement operation before and after changes in the law,and to explore its clinical significance.M ethods The voluntary acceptance of CRP and ESR testing department of orthopedics implant placement operation in 1600 cases,using a random number table is divided into A,B group,800 cases in each group,the A group patients before operation,after 1 days,3 days,5 days,7 days,14 days and 30 days CRP index monitoring of patients in the B group,in the same time point ESR index monitoring,and the results were statistically analyzed.Results In the A group patients after third days CRP index reached its highest value declined gradually after 30 days after operation,CRP index decreased to normal values after third days;index of CRP were higher than that before the operation（P〈0.05）;but the preoperative and postoperative day 30 CRP index difference between groups was not statistically significant（P〈0.05）.Group B patients after seventh days ESR index reached its highest value declined gradually after 30 days after operation,ESR index decreased to normal;seventh days after operation ESR index were higher than that before the operation（P〈0.05）;but the preoperative and postoperative day 30 ESR index difference between groups was not statistically significant（P〈0.05）.Conclusion The level of plasma CRP in patients with postoperative detection for rational use of antibiotics and early postoperative infection screening provides reliable basis,has significant clinical value.
