电视胸腔镜辅助小切口治疗自发性气胸

目的总结电视胸腔镜辅助小切口（video—assisted mini—thoracotomy，VAMT）治疗自发性气胸的体会。方法63例自发性气胸患者，均行VAMT。对于直径≤1cm的9例肺大疱行单纯结扎，对直径〉1cm的39例行切除，对未发现明显肺大疱的15例行肺尖部分切除。结果全部手术均顺利完成，无严重并发症发生。术后有1例患者切口液化，1例肺尖部切除范围较大者出现胸顶部少量积气，经对症处理后愈合良好。结论VAMT肺大疱切除治疗自发性气胸具有操作简单、安全性高、损伤较轻等优点，值得推广应用。

Application of Video-assisted Mini-thoracotomy for Spontaneous Pneumothorax

Objective To evaluate video-assisted mini-thoracotomy （VAMT） for spontaneous pneumothorax. Methods 63 patients of spontaneous pneumothorax underwent VAMT. 9 cases with diameter ≤ 1 cm were ligated with silk thread, 39 cases with diameter 〉 1 cm underwent bunamiodyl resection, and the other 15 cases without bunamiodyl underwent apical lung partial resection. Results There was no massive hemorrhage, no changing to thoracotomy, no blood transfusion, nor death in surgery. Only one ease occurred incision liquefaction, and one case of small amounts pneumatosis at the thorax top. Conclusion It is safe, economical and little hemorrhage of VAMT to the spontaneous pneumothorax.
