糖尿病患者血栓前状态的实验观察

为观察135例Ⅱ型糖尿病患者的血栓前状态，检测其凝血像（包括9种凝血和纤维蛋白溶解指标）、血小板聚集率（腺苷二磷酸、血小板活化因子和胶原诱导法）以及自然全血法血栓弹力图技术检测糖尿病患者血液凝固性的改变。结果全组有20％～22％患者的凝血像呈高凝状态，与对照组100例比较无显著性差异（P＞0．05）；122例血小板聚集率明显增高者占58．2％，与对照组100例相比有显著性差异（P＜0．05）；55例自然全血血栓弹力图的r值和k值明显缩短、ma幅度明显增宽者占70．9％的病例，与对照组50例相比有显著性差异（P＜0．05）。结果表明血小板聚集率和自然全血血栓弹力图可作为糖尿病患者血栓前状态的筛选试验。

Laboratory Observation of Prethrombotic State on Diabetes Mellitus

Objective:To investigate the prethrombotic state in 135 patients with noninsulindependent diabetics. Methods: The coagulation profiles including 9 indices of coagulation andfibrinolysis tests, platelet aggregation tests by addition of ADP, PAF and collagen-induced, andthrombelastography-tracing with natural blood. Results: The coagulation profiles showedhypercoagulation state about 20% to 22% in 135 cases, the platelet aggregation rates increasedabout 58. 2% to 71. 3% in 122 cases, and the TEG-tracings showed r (reactive ) and k(coagulative) time shortened and ma-wide broadened about 70. 9% in 55 cases. The differences inthese 3 groups of laboratory examines as compared with controls were P >0. 05, P <0. 05 and P<0. 05,respectively. Conclusion: Based on these data, we considered that platelet aggregation andnatural blood TEG may be the simple screening tests in prethrombotic state of diabetics.
